Death of the Member

Upon the death of the Plan Member, benefits may be payable to the beneficiary of the Member’s Plan, as designated by the Member, prior to his/her death.

If the beneficiary on file with the Plan Administrator is the spouse of the deceased Member, the spouse should use the Statement of Claimant – Spousal form to advise the Plan Administrator of the Member’s death and select the method of payment of any benefits that may be payable to the spouse.

If the beneficiary on file with the Plan Administrator is a person other than the Member’s spouse, the beneficiary should complete the Statement of Claimant – Non Spousal to advise the Plan Administrator of the Member’s death and arrange for the payment of any benefit that may be required under the provisions of the Member’s Pension Plan.

If the beneficiary on file with the Plan Administrator is the Member’s estate, the executor of the estate should complete the Statement of Claimant – Estate to advise the Plan Administrator of the Member’s death and arrange for the payment of any benefit that may be required under the provisions of the Member’s Pension Plan.

Completed forms should be sent to the Plan Administrator.
